چکیده

Abstract Background Pregnant women with Inflammatory Bowel Disease (IBD) continue thiopurines to maintain remission. Intrahepatic cholestasis of pregnancy (ICP) is the most common liver disorder pregnancy, often causing distressing maternal symptoms and potential implications for outcome. Case studies have reported treatment-resistant intrahepatic in thiopurine-exposed IBD pregnancies. We aimed investigate whether are associated an increased risk ICP course setting thiopurine therapy. Methods Single-centre retrospective cohort study comparing incidence exposed vs non-exposed patients compared age-matched pregnant controls. Results The consisted 386 pregnancies 243 IBD, controls without IBD. In was more amongst (n=10 9.0% n=5 1.8%; OR 5.34 [1.78-16.02]; p=0.021). Only exposure development ICP. Age, disease activity, phenotype, previous surgery, mesalazine, biologic, steroid were not associated. All cases (of which 4 received ursodeoxycholic acid) improved symptomatically discontinuation. significantly lower (n=5 1.3% n=15 4.1%; p=0.039). Thiopurine-exposed had higher odds experiencing non-IBD (n= 10 1.3%; 7.55 [2.52-22.57]; p<0.001). a comparable (n-=5 1.8% p=0.75). Severe (total serum bile acids ≥40 µmol/l) occurred 80% patients, 40% (p=0.25), 20% (p=0.09). found no evidence unfavourable cases. Median week delivery different among all three groups (p=0.39). Conclusion Thiopurine general population Clinicians involved management should be aware assess when occur. Prospective required understand relationship between metabolites

Intrahepatic cholestasis of pregnancy.

Intrahepatic cholestasis of pregnancy is the most common pregnancy-specific liver disease that typically presents in the third trimester. The clinical features are maternal pruritus in the absence of a rash and deranged liver function tests, including raised serum bile acids. Intrahepatic cholestasis of pregnancy is associated with an increased risk of adverse perinatal outcomes, including spon...

Fatal intrahepatic cholestasis associated with triazolam.

A 44-year-old man developed severe pruritus with jaundice which subsequently proved fatal. Liver histology showed intense cholestasis, but at postmorten the bile ducts were patent and there was no cirrhosis, the findings being consistent with a cholestatic drug reaction. The most likely precipitant was the benzodiazepine triazolam, and surveillance is indicated for any further reactions to this...

ATP8B1 mutations in British cases with intrahepatic cholestasis of pregnancy.

BACKGROUND Intrahepatic cholestasis of pregnancy (ICP) affects approximately 0.7% of pregnancies in the UK and is associated with prematurity, fetal distress, and intrauterine death. Homozygous mutations in the ATP8B1 gene cause cholestasis with a normal serum gamma-glutamyl transpeptidase (gamma-GT), and have been reported in two forms of cholestasis: progressive familial intrahepatic cholesta...

Alanine aminotransferase as a predictor of adverse perinatal outcomes in women with intrahepatic cholestasis of pregnancy

OBJECTIVE To evaluate the associations between adverse perinatal outcomes and serum transaminase levels at the time of diagnosis in patients with intrahepatic cholestasis of pregnancy. METHODS We performed a retrospective analysis of patients hospitalized for evaluation of intrahepatic cholestasis of pregnancy from January 2013 to June 2014 in a tertiary center. Seventy-one patients were divi...

A Comprehensive Evaluation of Steroid Metabolism in Women with Intrahepatic Cholestasis of Pregnancy

Intrahepatic cholestasis of pregnancy (ICP) is a common liver disorder, mostly occurring in the third trimester. ICP is defined as an elevation of serum bile acids, typically accompanied by pruritus and elevated activities of liver aminotransferases. ICP is caused by impaired biliary lipid secretion, in which endogenous steroids may play a key role. Although ICP is benign for the pregnant woman...
